Respected Hajj Pilgrims of Trishal Upazila:

Please download the attached medical check-up form, come to Trishal Upazila Health Complex with 2 copies of passport size photo, fill up the respective fields of the form and submit to the Medical Board with the following documents:

1. COVID-19 Vaccination Certificate

2. Health Form (Including Hajj Tracking Number)

3. Blood Group & Rh Type

4. RBS Report

5. Urine R/M/E

6. ECG Report

7. Chest X-Ray P/A View (100% Digital)


After check-up by the distinguished Medical Board, Candidates should submit all these documents to Mymensingh Civil Surgeon's Office for Meningitis & Influenza Vaccination
